Report Name: Employer's Contribution & Payroll Report

Who's Required to Submit: Every employing unit determined liable under Iowa's unemployment insurance laws.

Where to Send Report: Employers should mail completed reports to, Iowa Workforce Development, Attn:  Tax Bureau, 1000 East Grand Avenue, Des Moines, IA 50319-0209. It is unnecessary to include a paper copy of the quarterly Employer’s Contribution & Payroll Report when you have filed through the UITS or by CD using UITS.

Comments: This form is to be completed for each calendar quarter, even when there is no employment in a specific reporting quarter. The Payroll Listing portion of the report may be submitted through the UITS Online Reporting System,  the paper version of the quarterly Employer's Payroll Continuation Report or by CD.

A Summary of Quarterly Payroll By Location (form # 65-5305) should be submitted with the report by all employers who have requested and been granted the assignment of a location code for various separate and distinct establishments for the specific purposes of mailing a Notice of Claim filing.

The Multiple Worksite Report (form # BLS-3020) can be submitted with the paper version of the quarterly Employer's Contribution & Payroll Report by all employing units who operate the same business activity in more than one location or conduct different business activities from one or more locations within Iowa.   Please note: if you choose to file the Employer's Contribution & Payroll Report through the UITS Online Reporting System, you will need to complete and mail the BLS-3020 form and the Summary of Quarterly Payroll by Location (form # 65-5305) under separate cover.
