OSHA Fatality/Catastrophe Reporting Requirements
Iowa’s Occupational Safety and Health Act (IOSHA) requires
reporting for work-related incidents resulting in the death of an employee or in-patient
hospitalization of three (3) or more employees. The employer must orally report such
incidents in person or by phone within 8 hours of the incident.
Employers can call (877) 2IA-OSHA - (877)
242-6742 to make these
reports. The line is available 24 hours a day, including weekends and holidays. To
report in person, employers can go to the Iowa Division of Labor at 1000 East Grand
Avenue, Des Moines, Iowa.
